Preliminary Questions
Please read all of the following questions before completing a proposal.

Is the proposed activity within the scope of IACUC review?
- Will the animal activity use live, vertebrate animals?
If not, the animal activity is not within IACUC’s purview. (E.g., use of live, invertebrate animals or chick embryos.) - Will the animal activity involve research, instruction, testing or exhibition?
If not, the animal activity is not within IACUC’s purview. (E.g., use of goats by Facilities to eat poison ivy on campus.) - Is the animal activity to be conducted on Haverford’s campus or to be supported with funds administered by the College?
If not, the animal activity is almost certainly not within IACUC’s scope of review. Consult lstronglee [at] haverford.edu (Provost Linda Strong-Leek).
- Will the animal activity use live, vertebrate animals?

PI and training
- Is the person who will conduct the animal activity on campus or make use of the funds a Haverford faculty member?
If not, a Haverford faculty member must serve as Principal Investigator (PI) for the animal activity to be allowed. - What training must the PI complete prior to protocol submission?
The PI must document having passed the exam for the CITI course "Working with the IACUC" and additional modules as specified by the IACUC Chair. Upon completion of each course / module, the user will receive a certificate that must be forwarded to the IACUC Chair.
- Is the person who will conduct the animal activity on campus or make use of the funds a Haverford faculty member?

Location of animal activity
- Will the animal activity be conducted in the field?
If so, IACUC must ensure that all local, state, and federal regulations are followed. Relevant regulations must be appended or linked to the PI’s protocol review form. The PI must contact msweeney [at] haverford.edu (Mark Sweeney) to perform an OHS review. - Will the PI use only free-living wild animals?
If so, such studies are exempt from IACUC review and approval if they take place in the natural habitat of the animals, and they neither involve an invasive procedure, nor harm the animals under study, nor materially alter the behavior of the animals under study. IACUC uses the protocol review form to verify exemption, in which case 4-8 and 10 are ignored.
- Will the animal activity be conducted in the field?

Privately owned animals
- Will the PI use any privately owned animals?
If so, the PI must draft a consent form and submit it with the protocol review form. Sample forms are available on request.
- Will the PI use any privately owned animals?

Participants in animal activity
- Will any Haverford or Bryn Mawr faculty, staff, or students participate in care and use of the animals?
If so, the PI must describe qualifications and training of such personnel, except Rodent Facility employees and manager, in the protocol review form and must document all training. All such personnel, except private owners of the animals, must attend training sessions with our veterinarian. - Will any personnel other than Bi-College faculty, staff, or students participate in care and use of the animals?
The PI must describe qualifications and training of such personnel, other than Haverford’s veterinarian on IACUC, on the protocol review form and must document all training.
- Will any Haverford or Bryn Mawr faculty, staff, or students participate in care and use of the animals?

Facilities
- Will the PI use any facilities at Haverford College?
If so, the PI must ask our jhfried [at] gmail.com (veterinarian) to ensure that the facilities are appropriate and inspected prior to use. If the Rodent Facility will be used, all personnel who participate in care and use of the animals must submit a risk assessment questionnaire, complete lab safety training, and complete the appropriate CITI course(s) before the protocol is reviewed. Before approval, the PI must designate two individuals to act on the PIs behalf in the event of an emergency. - Will the PI use any facilities at a performance site other than Haverford College?
If so, the performance site must be at an Assured institution for the animal activities to be allowed. The PI must aid IACUC to secure a memorandum of understanding that specifies how oversight responsibility is shared among IACUCs.
